  • Minnesota residents wishing to receive copies of driving records must follow a relatively simple process. First, you must fill out a Record Request form. When the form is completed, the DVS will review to find out if you should have access to the record in question. The applicant has the choice of submitting the form by mail or applying in person at the DVS office in St. Paul.
